Travel Insurance Enrollment (Challenge Game)

  • Please list ALL players from the Club or team travelling to the tournament in ONE form. 
  • To add additional rows click the + sign at the end of the row
  • The form will total the amount owing. Once submitted, CGAA will issue an invoice to the Club to collect payment. CGAA will not collect payment from each individual player.
  • The number of days to purchase should be the days the player is travelling and playing in the tournament. If the player is travelling for personal reasons before or after the tournament, separate travel insurance should be purchased.
  • This insurance if for CGAA members who are playing in the tournament, not for family members travelling to the event
  • The submitter of this form is responsible for the accuracy of the submissions. Neither CGAA nor the insurance provider will verify the accuracy of the submissions for travel insurance.

    Travel Start date is the day the insured physically leaves their residence for the trip, that is the effective date the Travel Insurance coverage begins.Medical Travel Insurance is sold and made available for specified consecutive days while travelling out of Canada. Insurance is not provided on an intermittent basis or for periodic intervals.
