  • Travel Start date is the day the insured physically leaves their residence for the trip, that is the effective date the Travel Insurance coverage begins. Medical Travel Insurance is sold and made available for specified consecutive days while travelling out of Canada. Insurance is not provided on an intermittent basis or for periodic intervals. 

  • This insurance if for CGAA members who are playing in the tournament, not for family members travelling to the event

  • The submitter of this form is responsible for the accuracy of the submissions. Neither CGAA nor the insurance provider will verify the accuracy of the submissions for travel insurance.
